• Conduct comprehensive patient assessments to determine health status and care needs.
• Develop and implement individualized care plans based on assessment findings.
• Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
• Monitor patients’ progress and response to treatment, making adjustments as necessary.
• Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
• Educate patients and families on health management, disease prevention, and wellness strategies.
• Maintain accurate documentation of patient care activities in compliance with regulatory standards.
• Utilize electronic health record systems such as eClinicalWorks for efficient data management.
