Job Title: Youth Home Coordinator

JOB SUMMARY

The CYS Youth Home Coordinator oversees the day-to-day operations of a 24-hour staffed home for children and youth placed under the Specialized Homes and Supportive Services (SHSS) program, funded by the Ministry of Children and Family Development (MCFD). In this role, the Coordinator works directly with the participant and ensures all health and safety standards are met, while monitoring the overall health and well-being of the child/youth in the program.
The Coordinator provides ongoing supervision, training, coaching, and mentorship to staff working with the child/youth to ensure consistency of care. This role also supports the Team Leader in the recruitment and onboarding of new staff.
The coordinator is responsible for establishing, delivering, and enabling the young person’s SHSS Service Plan, and ensures that service goals are monitored and evaluated regularly in collaboration with the Circle Care Team.

QUALIFICATIONS

  • Diploma (2-year) in a related human or social services field.
  • Minimum of three (3) years of recent related experience.
  • At least one (1) year of supervisory or administrative experience.
  • Specific training in behavioural modification for children with support needs, attachment-based parenting, and relationship-based interventions (trauma-informed practice) is considered an asset.
  • An equivalent combination of education, training, and experience may be considered.
  • Proficiency in computer use and documentation.
  • Valid driver’s license and access to a reliable vehicle.
  • This position requires flexibility to work on days, evenings, and weekends as needed.
  • Demonstrated understanding of trauma-informed, culturally safe, and inclusive practices in working with children and youth.
  • Strong interpersonal, communication, and problem-solving skills, with the ability to lead and support a team in a dynamic care environment.
Responsibilities

Leadership & Staff Support

  • Under the guidance of the Team Leader, collaborates with other team members to provide orientation, training, work direction, and guidance to CYS Supportive Living Workers.
  • Clarifies existing policies, reviews staff performance, and improves operations to enhance participant development.
  • Provides leadership in key service areas to ensure consistent and effective delivery:
  • Goals and strategies
  • Monthly check-ins
  • SHSS Service Plans
  • Daily Logs and Documentation
  • Facilitates and schedules Care Circle Meetings
  • 90 Day Meetings with MCFD
  • Ensures Service Delivery standards are met
  • Assists with scheduling and responds to immediate staffing needs.
  • Participates in the on-call rotation, which may include responding to after-hours calls related to emergency services, critical incident reporting (as per OneSky and MCFD policy), emergency repairs, and staff absences.
  • Ensures all staff interactions reflect trauma-informed, culturally safe, and relationship-based practices.

Program Oversight

  • Oversees the day-to-day activities of staff and the residence.
  • Documents, implements, and provides input into the evaluation of the program.
  • Provides feedback to the Team Leader for staff performance evaluations and development.

Participant Support

  • Provides life skills training such as meal preparation, housekeeping, personal care skills, and personal finance, and supports implementation of personal service plans.
  • Assists participants with activities of daily living including feeding, hygiene, grooming, and toileting.
  • Participates in participant-focused activities in accordance with care plans.
  • Administers medication to participants in accordance with established policy.
  • Assists with case management by identifying potential problems and reporting challenges to the Team Leader.

Collaboration & Planning

  • Collaborates with specialized or professional partners (e.g., behavioral analysts, speech and language professionals, teachers) to develop appropriate program plans and achieve participant goals.
  • Facilitates and schedules Care Circle Meetings and 90-Day Reviews with MCFD.
  • Coordinates and collaborates with educational support staff to monitor and support the youth’s Individualized Education Plan (IEP) or learning goals.
  • Works in collaboration with the service team and family toward shared goals of transition or reunification.

Documentation & Compliance

  • Maintains all required documentation as per service delivery standards on the SHSS Portal.
  • Ensures service delivery standards and health and safety regulations are met.

Community Engagement & Residence Maintenance

  • Identifies and connects participants with social, economic, recreational, and educational services in the community that support their goals, including peer relationships and social inclusion.
  • Performs light residence maintenance and housekeeping duties including laundry, sweeping, mopping, lawn care, inventory, shopping, and food services.
