Initials: _______ ORGANIZATION FLOW: This position reports to the Executive Director and serves as the programmatic department head for the day services department. MAJOR RESPONSIBILITIES: Direct the Utah Day Services Department (DSG, DSI, DSP and summer program) Supervise and train coordinators and staff assigned to the Dept. Ensure compliance of the PCSP process and other contract requirements through internal auditing process. Assist in the intake process of new day service consumers. Maintain a small caseload for specialized consumer situations. Maintain professional relationships with DSPD, school districts, funding agencies, state, and association personnel. Team player responsibilities Other duties as assigned. SPECIFIC DUTIES: 1. Direct the Utah Day Services Department (DSG, DSI, DSP and summer program) Oversee day-to-day operation of Day Supports department. Manage staffing of department. Meet with coordinators weekly or as needed to review operation of Day Supports department. Oversee service utilization and request additional funding as needed. Oversee billing of department services. Oversee incident reporting process for department. Assist assigned staff in addressing consumer issues as they occur and facilitate resolution Administer the department staff incentive budget. Review dept human rights plans for consumers as needed to ensure plans meets requirements for presentation to the Human Rights Committee. Supervise and train coordinators and staff assigned to the Department Oversee training of coordinators. Provide support and evidence-based training to staff assigned to the department. Conduct evaluations with staff annually or as needed. Assist the Human Resource Department and Executive Director with hiring, disciplinary action and terminations. Provide oversight to coordinator specialties. Delegate responsibilities to team members as needed. Ensure policies and procedures related to the area of specialty are met and provide a progress report on specialty areas as needed Provide counsel and support to assigned staff as needed. 3. Ensure compliance of the PCSP process and other contract requirements through the internal auditing process. Conduct quality enhancement reviews of coordinators through attendance at PCSP’s, reviewing support strategies and progress notes as needed. Conduct adherence monitoring activities as needed. Assist in quality assurance activities for contract compliance for local, state and federal audits. Review compliance for medication logs and incident logs and report trends in the data to the Executive Director 4. Assist in the intake process of new day service consumers. Assure application forms/intake packets are complete Gather, analyze, interpret medical, psychological, educational, and other assessment information Interview consumers, parents or referring agency personnel to complete the intake Administer or cause to be administered other appropriate assessment instruments as needed for support planning or consumer assessment. Complete assessments within one month of consumer starting services/consumer’s annual PCP meeting. Recommend admission based on data and needs of the consumer 5. Maintain a small caseload for specialized consumer situations. Attend Person Centered Planning Meetings and facilitate as needed. Address annual paperwork at PCSP meeting. Prepare support strategies in cooperation with the consumer and their circle of supports. Develop support strategies in accordance with the person-centered support plan. (Support strategies must be completed within one month of the PCP meeting Progress Sheets (data sheets) are completed and distributed to direct care staff the first working day of the month following the month in which the PCP meeting was held. Track and record individual progress. (Progress reports to be completed by the 15th of each month Periodically (at least annually or as directed by the PCSP team) evaluate consumer progress, and review results with all team members. Make recommendations for PCSP revisions. Conduct assessments to develop support strategies as needed. Develop/update human rights action plans as needed to address consumer right restrictions 6. Maintain professional relationships with state/funding agencies, school districts, residential providers, association personnel, families and clients. personnel. Attend provider fairs and represent CETC in various settings including schools, other agencies and state meetings. Carry out communication and develop a positive working relationship with parents, providers and funding agencies in a professional manner. 7. Team Player Attend workshops, conferences, trainings and community meetings as directed. Be responsible to attend training meetings required to fulfill training requirements. All training and modules must be completed and submitted either before or on the due date given. Failure to meet training requirements may result in a work suspension.
