Employee Assistance, Wellness Benefits, Dependent Care & Long-Term Disability Insurance DESCRIPTION The Oregon Needs Assessment Assessor position takes the lead on all required state assessments of support needs for individuals experiencing intellectual and/or developmental disabilities (I/DD). The Adult Protective Services Investigator conducts protective service investigations for individuals enrolled in Developmental Disabilities programs in accordance with the Oregon Administrative Rules. The investigator authors all associated reports, makes determinations on the allegations, and identifies specific actions to avoid further abuse. This position serves the entire County listed and provides occasional/back-up coverage to all other counties supported by the CCS DD Program. SUPERVISION This position does not supervise any employees. SUPERVISION RECEIVED Works under the direction of the DD Regional Supervisor, DD Program Manager, and/or Chief Operating Officer. SUPERVISORY RESPONSIBILITIES This job has no supervisory responsibilities. RESPONSIBILITIES OREGON NEEDS ASSESSMENT COORDINATOR: Ensure the individuals served have the required Oregon Needs Assessment (ONA) completed to assist in acquiring services and resources needed to achieve goals and maintain independence in our communities. Support the CCS Developmental Disabilities team to establish timelines and ensure all required needs assessments are completed accurately and within state required timelines. Ensure OAR Guidelines are followed including but not limited to assessment process, timelines, and documentation. Responsible for maintenance, submission, and timeliness of all records, progress notes, reports, and all other required documentation. Coordinate assessment meetings with the individual, as well as family members, providers, case managers, and any other community partners and participants, as necessary. Understand the relevant sections of the Community Developmental Disability OARs in conducting the Oregon Needs Assessment (ONA). Attend the core competency training by DHS and all follow up trainings, phone conferences, and other mandatory trainings as directed by ODDS. Document all ONAs in the electronic health record progress notes. Use eXPRS billing and reporting system to record all ONAs. Communicate any needs, health and safety issues, or other information needed with services coordinators. Complete all needed file reviews, reviews of history, review of prior assessments, review documentation, complete face to face interviews, and participant interviews within the 30-day timeline per OAR. Complete other duties as assigned by supervisor. ADULT PROTECTIVE SERVICES INVESTIGATOR: A. Investigation and Assessment of Risk of Abuse/Neglect/Exploitation Gathers and assesses pertinent information about reports and makes appropriate decisions regarding priority for investigation. Investigates reports of abuse, neglect, or exploitation of adults with developmental disabilities. Provides information of APS investigation procedures to clients, caregivers, family members, staff, brokerages, and others. Appropriately uses diverse methods to elicit information from people with intellectual, functional or communication limitations. Conducts an assessment of the individual situation, including environment, physical health, mental functioning, and financial aspects. Identifies physical, emotional, and behavioral indicators of abuse, neglect, and exploitation. Assesses individual, family, and environmental contributors to abuse, neglect and exploitation. Uses relevant information in determining case finding. Plans with victims and others for immediate safety needs and takes emergency action if necessary. Develops effective relationships and coordinates investigations with law enforcement and the legal system, as appropriate. Regularly participates in local multi-disciplinary team (where one exists). B. Evidence gathering, exhibits, review of records and report preparation. When appropriate, prepares documentation and evidence to support protective services actions such as seeking and assisting with restraining orders, a protective order, court commitments, guardianship, and/or other court related activities. Understands basic principles of personal accounting sufficient to conduct investigation for purpose of safeguarding client's funds. Skilled in a variety of evidence gathering techniques including photography, review of medical/financial records, interviews by phone and in person. Effectively gathers information from clients, family members, health professionals, staff and others concerning suspected abuse/neglect/ exploitation. Understands and utilizes information for cases involving brokerages, independent providers, families, and clients. Upon completion of the investigation, prepares written findings, including any required actions to respond to abuse or prevent further abuse of the adult. Demonstrates through written report the findings of inconclusive, not substantiated or substantiated using the preponderance of the evidence standard of proof. Ability to utilize state required format for written reports. Clearly and comprehensively documents case findings in SERT and in written reports. Requirements QUALIFICATIONS Education and/or Experience OREGON NEEDS ASSESSMENT ASSESSOR: Must meet the minimum standards for a Service Coordinator/Case Manager under Community Developmental Disability Programs as defined in the Oregon Administrative Rule (OAR) 411-320-0030 and 411-415-0040 a) A bachelor's degree in behavioral science, social science, or a closely related field; OR b) A bachelor's degree in any field and one year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; OR c) An associate's degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; OR d) Three (3) years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, law enforcement, or housing. ABUSE INVESTIGATOR SPECIALIST: An abuse investigator specialist must have at least one of the following: (A) A bachelor's degree in human science, social science, behavioral science, or criminal science and two years of human services, law enforcement, or investigative experience; OR (B) An associate's degree in human science, social science, behavioral science, or criminal science and four years of human services, law enforcement, or investigative experience. Certifications This position has no certifications required. Other Skills and Abilities Must possess, or have the ability to learn, DD case management standards and the ability to implement them within the communities. Must have knowledge of, or the ability to learn about, DD services, resources, and funding sources. Must have the ability to effectively communicate both verbally and in writing and have the ability to work independently and complete all responsibilities in a thorough and timely fashion. Ability to represent the interests of the customer and the agency in a favorable light to the community. Must be computer literate and have excellent typing skills, have knowledge of basic office software programs and ability to effectively use such software. The position requires the handling of highly confidential information. Must adhere to rules and laws pertaining to client confidentiality. CRIMINAL BACKGROUND CHECKS Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 410-007-0200 through 410-007-0380. In addition to a pre-employment background check, each employee, volunteer, and contractor shall be checked on a monthly basis against the OIG and GSA exclusion lists, as well as other federal and state agency lists. If it is discovered that an employee, volunteer, or contractor is excluded or sanctioned it will be the cause for immediate termination of employment, volunteering, or the termination of the contract. PERSONAL AUTO INSURANCE Must hold a valid driver’s license as well as personal auto insurance for privately owned Vehicles utilized for CCS business such as client service purposes, travel between business offices and the community, to attend required meetings and trainings. Must show proof of $300,000 or more liability coverage for bodily injury and proof of $100,000 or more liability coverage for property damage and maintain said level of coverage for the duration of employment at CCS. The employee’s insurance is primary with CCS insurance being secondary. CCS reserves the right to deny any employee the use of a vehicle owned by CCS. PHYSICAL DEMANDS While performing the essential duties of this job, the employee is regularly required to use office automation including computer and phone system that require fine manipulation, grasping, typing, and reaching. The employee is also regularly required to sit, talk, listen, and use hands and fingers. The employee is also required to stand, walk, reach with hands and arms; stoop; kneel and/or squat. The employee may occasionally lift and/or move up to thirty pounds. Specific vision required by this position includes close, peripheral, distance, and the ability to adjust focus. This work also requires that the employee sustain their attention during periods of crisis. WORK ENVIRONMENT Tasks will occur both at the office and in the community. There will be travel required throughout the county(s) that the employee is assigned to. Occasional out of area travel and overnight stays will be required for attendance at meetings and/or trainings. Monitor and broker services at the home site of the customer, service provider, and office will be within a typical day’s routine. As indicated, employee also will be required to work in the community and homes of the customers we serve. Handicap access may not be available at all places where this position must go. This position may be required to respond to environments where a customer is in crisis. The environments in these situations are difficult to predict and may be in uneven terrain. This position may be exposed to the everyday risks or discomforts which require normal safety precautions typical of such places as an office.
