Your opportunity
Part-Time (4 days a week, Tuesday to Friday) – Based at Jannali
In this rewarding part-time role, you’ll provide specialist advice, advocacy, and practical support to people living with dementia and their carers. You will work 3 days in the South East Sydney Region and 1 day in the Illawarra Region, delivering individualised case management, facilitating education programs, and raising community awareness about dementia.

With empathy and expertise, you will:

  • Provide person-centred case management that supports dignity, choice, and wellbeing
  • Deliver evidence-based advice and advocacy during home visits and group settings
  • Conduct educational talks to increase understanding of dementia in the community
  • Complete timely assessments and care plans to ensure services are optimised and effective
  • Build and maintain strong relationships with clients, families, carers, and stakeholders
  • Manage budgets, maintain accurate client records, and ensure compliance with organisational standards

What you’ll bring
You’re a compassionate and knowledgeable professional who is passionate about making life better for older people and their families.

You’ll need:

  • Diploma in Community Services, Dementia Care, or equivalent qualification (essential)
  • Strong understanding of dementia, its progression, and its impacts on individuals and carers
  • Experience working with older people, particularly those living with dementia
  • Excellent communication, presentation, and advocacy skills
  • Proficiency in MS Office and client database systems
  • Current C Class driver’s licence and access to a reliable vehicle

Ideally, you’ll also have:

  • Experience in facilitating community education programs
  • Knowledge of aged care service systems and funding arrangements
