Your opportunity 18 Month Contract (Monday to Friday, 9am-5pm) – Western Sydney Area, Based at Rooty Hill In this rewarding full-time role, you’ll provide specialist advice, advocacy, and practical support to people living with dementia and their carers. You will be delivering individualised case management, facilitating education programs, and raising community awareness about dementia. With empathy and expertise, you will: Provide person-centred case management that supports dignity, choice, and wellbeing Deliver evidence-based advice and advocacy during home visits and group settings Conduct educational talks to increase understanding of dementia in the community Complete timely assessments and care plans to ensure services are optimised and effective Build and maintain strong relationships with clients, families, carers, and stakeholders Manage budgets, maintain accurate client records, and ensure compliance with organisational standards What you’ll bring You’re a compassionate and knowledgeable professional who is passionate about making life better for older people and their families. You’ll need: Diploma in Community Services, Dementia Care, or equivalent qualification (essential) Strong understanding of dementia, its progression, and its impacts on individuals and carers Experience working with older people, particularly those living with dementia Excellent communication, presentation, and advocacy skills Proficiency in MS Office and client database systems Current C Class driver’s licence and access to a reliable vehicle Ideally, you’ll also have: Experience in facilitating community education programs Knowledge of aged care service systems and funding arrangements Why Anglicare Sydney?
