About the Role We’re hiring a Nurse Practitioner (NP) or Physician Assistant (PA) to support our dementia care program. This hybrid role combines clinic-based and telehealth visits, with opportunities to expand into senior living communities. You’ll conduct cognitive evaluations, manage behavioral and chronic conditions, and educate families navigating dementia.
