What you will do: The Dental Treatment Plan Coordinator will support, advocate, guide and intervene on behalf of their patients and families to ensure successful implementation of the Care Plan.  This role acts as the primary interface between CHDC and other specialty offices and healthcare centers (hospitals).  Care coordinators will assist in providing a medical home for patients with chronic conditions or special health needs.  Support, guide, and develop and implement the Medical Home program by working with patients, their families and community resources. Duties include but are not limited to: * Develop and maintain relationships with patients to learn their specific dental needs and goals. * Welcome new patients and act as the Liaison between the patient and the Dentist and clinical team to better support the patient throughout the process. * Work with the Dentist and clinical team to organize a comprehensive treatment plan * Identify barriers to care from the patient and CHDC’s perspective * Patient coaching/counseling/education as needed * Provide Triage as needed * Present and Implement patient treatment plan of care, identifying variances and applying appropriate interventions as necessary * Determine insurance calculations and discuss financial options with patient * Utilizes established documentation standards to maintain quality of care plan documentation to include patient progress toward and barriers to achievement of care plan objectives/outcomes. * Develop communication and referral mechanisms to assure that there is seamless communication between CHDC and Dental specialist offices * Assist the patient in coordination of any additional tests, images and consults with specialists as deemed appropriate by the Dentist in developing the Care Plan * Assists dental front desk staff as needed * Other duties as assigned  Requirements: Qualifications, Education, and Knowledge * High school diploma or GED; completion of a formal education program in dental assisting strongly preferred; completion of on-the-job training in the field required.  Certifications and licensing as required by the state if performing Dental Assisting duties in a dual role. * Minimum 1-2 years dental office experience as office manager/dental assistant, patient coordinator, or similar role.  * Bilingual/bicultural preferred. * Attendance required at assigned CHDC site(s)  CHDC is an EOE/Veterans/Disabled/LGBTQ+ employer.
