Description Connecticut Institute For Communities, Inc. / CIFC Health seeks to hire a full-time Dentist (DDS or DMD) licensed to practice in the State of Connecticut. Under the direction of the Chief of Dental Services, this position is responsible for performing General Dentistry duties, including making appropriate referrals and follow-ups. Bilingual candidates are strongly encouraged to apply. In addition, as a federally qualified healthcare center (FQHC), this position is eligible to apply for a Loan Repayment Program through HRSA (Health Resources and Service Administration). Requirements 1. Take a complete patient history, complete a thorough examination, administer injections, perform dental procedures, order tests, and record pertinent data within the electronic medical records. 2. Make a dental diagnosis and institute treatment, and/or institute a referral. 3. Treat patients with the accepted standard of care in the State of Connecticut. 4. Return patient phone calls. 5. Maintain a positive working relationship with all staff in all departments. 6. Maintain strict patient confidentiality. 7. Participate in appropriate staff meetings and training sessions. 8. Provide all functions, as directed, within the individual Physician’s licensing and credentialing authority, never exceeding such authority. 9. Complete timely all related patient and administrative paperwork (whether manually or electronically) as required. 10. On an annual basis provide at least the specified number of qualified patient visits at 1.0 FTE (pro-rated for less than 1.0 FTE), as detailed in the “Provider Compensation Incentive Program and Policy” as adopted by the CIFC Board of Directors, as the same may be revised from time to time. 11. As part of the Physician’s base activities and included in base compensation, participate on a pro-rata FTE basis in the dental (as applicable) coverage programs of the Health Center. 12. Comply with the Health Center and CIFC administrative rules, regulations and Policies and Procedures as in effect, and as the same may be amended from time to time. 13. Provide after-hours advice. 14. Ensure preferred language is offered to patients, and provide educational items in their preferred language. 15. Review and record measures as required for UDS and PCMH guidelines . 16. Adhere to guidelines for PCMH credit. 17. Ensure care plan allows for patient lifestyle and choices, and assure patient understands care plan. 18. Incorporate self-management into the office visit. 19. Provide patients with self-help tools. 20. Other related duties as assigned.
