Description

To perform a clinical assessment of patients attending the SDEC and act appropriately on their findings. To promote a cohesive multi-disciplinary team both within the SDEC , ICC Pathway and integrated services, including supporting other teams within Oxford Health NHS Foundation Trust. In conjunction with the Clinical Lead, contribute to the monitoring and auditing of the standards of care in the SDEC and developing and implementing arising action plans. To assist in the development and implementation of policies, protocols and guidelines relevant to the SDEC, including recruitment, probation, performance management and sickness absence management.
To improve the quality of care received and the experience of patients and their carers through implementation of initiatives which reflect the promotion of independence and the National Service Framework for Older People.
