Applying for this Deputy Therapy Manager role, will be a prime opportunity to develop your leadership and service development skills, as a dynamic, problem solving Occupational Therapist or Physiotherapist.
The integrated discharge hub (IDH) at Milton Keynes University Hospital is responsible for being a single point of contact for professionals, providers, patients and their families to support safe and timely discharge. The IDH has an integrated health and social care single line management structure overseeing professionals from a variety of services which are integral to supporting patient discharge, for which you will actively lead and manage organisational day to day service provision.
The ethos of the hub is to provide a seven-day service, ensuring patients are central to all discharge planning and they, along with their families are kept updated through their discharge journey and seamlessly returned to their chosen residential setting. The hub is responsible for the safe and timely discharge of patients from Milton Keynes Hospital, reducing delays in discharge by coordinating professionals to ensure the necessary support and services are in place.
Be the key point of contact in coordinating patient discharges for patients requiring therapy, being the expert advisor and developing and maintaining access to other services that support patient discharge.
Share key knowledge and skills to Discharge Practitioners.
Provide operational leadership to members of staff within the IDH and direct supervisees within the IDH and CNWL Home first as required. This includes management of HR and organisational requirements.
Responsible for ensuring that members of staff within the IDH are carrying out the daily operational delivery of all discharge and admission avoidance processes in line with local and national guidance and in collaboration with system partners.
To deputise at key stake holder meetings and events, exploring opportunities to improve discharge planning and admission avoidance.
Be accountable to your own professional organisation working in collaboration with a range of health and social care professionals to ensure safe, sustainable and seamless discharges for patients at Milton Keynes University Hospital and for patients requiring repatriation to Milton Keynes.
Specifically promote the inclusion of patient communication within all work undertaken by the IDH, to ensure that patients/families/carers views are included within discharge planning and are kept updated on the progress of discharge planning.
Maintain productive partnerships with other health and social care organisations, including the voluntary sector.

To be professional and legally accountable for all aspects of your work including the case management of patients in your care.
To use specialist professional and clinical knowledge across a range of procedures based on a sound knowledge of evidence-based practice and treatment options, using clinical assessment, reasoning skills and knowledge of treatment skills.
To undertake comprehensive holistic assessment of patients to determine suitable management of their rehabilitation needs in discharge planning support.
Identify all new patients and have an understanding from admission of any therapy requirements that may be needed on discharge and commence discharge planning.
Use clinical reasoning skills to risk assess appropriate therapeutic intervention response times for patients referred to Planned care therapy.
To assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
Use a range of verbal or non-verbal communication tools to communicate effectively with patients, relative’s, carers and other health and social care professionals to progress discharge plans as required. This will include patients who may have difficulties in understanding or communicating.
To be able to prescribe, order and review the equipment needs for patients requiring supported discharge.
On completion of risk assessments, be able to make recommendations to the multi-professional teams, about the level of support required to meet individualised packages of care on discharge.
To be able to promote a risk enabling approach to ward staff to encourage therapeutic intervention and functional activities, preventing deconditioning in the hospital environment and reduce length of stay.
To be responsible for the safe and competent use of all appropriate equipment.
Work collaboratively with a range of healthcare professionals from MKUH, CNWL, Social care and other organisations, to ensure needs led comprehensive treatment plans are in place.
Provide a comprehensive and highly specialist level of communication / liaison between MKUH, CNWL, Social care and other organisations to promote patients therapy requirements in treatment and discharge planning decisions.
Provide expert advice to the bed/site management team in relation to transfer/care issues.
To attend the appropriate ward rounds and multidisciplinary meetings as required.
Ensure accurate electronic records are maintained effectively, in a timely manner.
To work with staff from the Planned Care service line, to deliver seamless therapeutic treatment plans in the transition from hospital to home.
Take part in service audit to assess the effectiveness of Planned Care service line
To work collaboratively in partnership with CNWL, MKUH and BLMK ICB to implement system flow response as part of escalation processes.
To actively seek patient and carer feedback, to enable informed decision-making when reviewing service interventions.
Partake in clinical audit activities that impact on barriers to discharge.
Participate in research as appropriate.
Utilise the Patient Discharge Unit and provide advice, support and training where required
