The Designated Coordinator complies with the Support Plans for individuals receiving services according to federal and state regulations and the organization’s policies and procedures to ensure the quality delivery of services to all individuals served. JOB RESPONSIBILITIES: The Designated Coordinator job responsibilities include: 245D compliance and paperwork Train, schedule, and provide ongoing support and evaluation of direct care staff Manage services (which may be hourly, or 24-hour supports) Evaluate of the effectiveness of service delivery, methodologies, and progress on the person's outcomes Communicate with individuals, guardians, case managers, behavior analysts, and other team members to decide on services needed to support each individual most effectively Maintain and summarize data for reports Coordinate appointments for individuals as requested Problem solves when emergencies arise Work direct care as needed Will be required to be in an on-call rotation Requirements ***Please read the position qualifications below. This is a high-level Designated Coordinator position with additional education or experience requirements. Only resumes that have the required education and experience will be reviewed*** APPLICANT REQUIREMENTS: To qualify for the position, applicants must: Pass a Department of Human Services background check (Required) Have a current Driver’s License (Required) Work independently and have strong organizational skills Demonstrate strong written and oral communications skills Proficiently use Microsoft Office and Teams Have experience and understanding of 245D compliance EXPERIENCE/EDUCATION: *Only resumes that meet the required education and experience will be reviewed Must live in Minnesota and be willing to commute to the corporate office in Saint Paul, MN as well as sites throughout the metro area. 2 years’ experience working with individuals that have high behavioral support needs, preferred Minimum of 1 year of DC, Program Management/Supervisory experience, preferred Qualified candidates must have one of the following: Bachelor's Degree in a field related to human services, education, or health and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children; or an Associate Degree in a field related to human services, education, or health and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older, or equivalent work experience providing care or education to vulnerable adults or children; or a Diploma in a field related to human services, education, or health from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older or equivalent work experience providing care or education to vulnerable adults or children; or a Minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified above.
