As Nurse Practitioner will provide specialized care and management for patients with acute and chronic wounds, including but not limited to pressure ulcers, diabetic foot ulcers, venous ulcers, and post-surgical wounds. The NP will work collaboratively with a multidisciplinary team to develop and implement individualized care plans, perform wound assessments, and ensure optimal patient outcomes. • Patient Assessment and Care Planning • Conduct comprehensive patient assessments, including health histories and physical examinations. • Develop, implement, and evaluate individualized treatment plans for patients with complex wound care needs. • Order and interpret diagnostic tests related to wound care. • Prescribe medications and treatments as appropriate. • Wound Management • Perform wound debridement, dressings, and other advanced wound care procedures. • Utilize evidence-based protocols and guidelines for wound management. • Monitor and document the progress of wound healing, adjusting care plans as necessary. • Patient Education and Support • Educate patients and their families on wound care techniques, prevention strategies, and lifestyle modifications. • Provide support and counseling to patients and families regarding wound management and treatment plans. • Collaboration and Coordination • Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure comprehensive patient care. • Participate in multidisciplinary team meetings and case conferences. • Coordinate referrals to other healthcare providers or specialists as needed. • Quality Improvement and Professional Development • Participate in quality improvement initiatives related to wound care. • Stay current with advancements in wound care practices through continuing education and professional development. • Contribute to the development and revision of wound care protocols and policies. • Documentation and Compliance • Maintain accurate and up-to-date patient records, including assessments, treatments, and outcomes. • Ensure compliance with all regulatory and accreditation standards related to wound care. Qualifications Qualifications Education: • Master’s or Doctoral degree in Nursing from an accredited program. • Current licensure as a Nurse Practitioner in MA. • Certification in Wound Care (e.g., WCC, CWON, CWS) preferred. Experience: • Experience in an acute care or outpatient setting preferred. Skills and Competencies: • Strong clinical assessment and decision-making skills. • Proficiency in wound care techniques and procedures. • Excellent communication and interpersonal skills. • Ability to work independently and as part of a multidisciplinary team. • Strong organizational and time management skills. • Commitment to maintaining continuing education credits for both Advanced Practice License and Wound Care Certifications. Physical Requirements: • Ability to perform tasks requiring physical exertion, including lifting patients and moving equipment. • Ability to stand, walk, and remain on feet for extended periods. • Manual dexterity to perform wound care procedures.
