Here's what you'll be doing We’re looking for an experienced Diabetes Care Specialist to join our team. You’ll be responsible for patient experience and success of our practice, including: Counsel and educate patients and families appropriately on diabetes care in collaboration with the care team Complete nutritional assessments and make appropriate recommendations as a part of the care planning process Analyze CGM data and translate to treatment changes for patients Communicate with patients through video calls, in person meetings, telephone and electronic messaging Create and maintain documentation Ensure patient satisfaction by building rapport and maintaining positive relationships with patients and their families Other duties as assigned What you need to be successful Registered Nurse (RN) or Registered Dietitian (RD) and Certified Diabetes Care and Education Specialist (CDCES) required Licensed or registered in the state of practice as required by state regulations Prior experience leveraging CGM data Passion for the impact of your work on the broader world, and excitement to help reduce the burden of chronic disease We want you to know Ryse Health is an Equal Opportunity Employer.
