Assist patient with accessing community-based resources by supporting them with applications, transportation needs and follow up on any referrals made to ensure patient utilizes identified resources. Enhance patient’s ability to effectively communicate with healthcare provider Works with staff and providers to facilitate a care plan and communicates with them regarding the status of efforts Support patient understanding of educational materials and resources about diabetes Qualifications CMA, LPN, Pharmacy Technician, EMT/Paramedic Diabetes Community Care Coordinator Certification required within 60 days of hire Community Health Worker certification within 90 days of hire Mandatory Reporter certification
