Responsibilities:
Provide professional consultations for USA Health patients with Diabetes Mellitus Type 1 & 2 and Amyotrophic Lateral Sclerosis (ALS). The ideal candidate will have experience in managing complex nutritional needs, including dysphagia, weight management, glycemic control, and enteral nutrition.
This hybrid role for a Registered Dietitian consists of 32 hours per week at USA Health Providence Diabetes Center, providing diabetes education and counseling to patients with Diabetes and 8 hours per week at the Kelly Butler ALS Center, located within the USA Health Strata Center, supporting the nutritional needs of patients with Amyotrophic Lateral Sclerosis (ALS).

DELIVER EDUCATION ON KEY TOPICS INCLUDING:

Diabetes pathophysiology
Medication and insulin administration
Nutrition therapy and weight management
Glucose monitoring and continuous glucose monitoring (CGM) technology
Physical activity
Problem-solving strategies
Management and prevention of hyperglycemia and hypoglycemia
Complication prevention and foot care
Coping strategies for living with diabetes
Evaluate patient progress toward short- and long-term goals and plan follow-up interventions accordingly.
Ensure accurate documentation of provider referrals in the electronic health record (EHR) and proper billing in accordance with Physician Group policies

Responsibilities

RESPONSIBILITIES FOR THE DIABETES CENTER:

Assist in the development and implementation of the Diabetes Self-Management Education program in alignment with the American Diabetes Association (ADA) Recognized Program Standards of Care.
Provide outpatient education and motivational health coaching to individuals with Type 1, Type 2, and Gestational Diabetes, both individually and in group settings, as directed by the Endocrinologist or Provider.

RESPONSIBILITIES FOR THE KELLY BUTLER ALS CENTER:

Serve as an integral member of the multidisciplinary team providing care to patients with Amyotrophic Lateral Sclerosis (ALS).
Participate in multidisciplinary rounds to support coordinated patient care.
Collaborate with providers to deliver individualized nutritional support and counseling to patients and their families/caregivers.
Conduct comprehensive nutritional assessments and develop care plans aimed at preventing weight loss in ALS patients.
Evaluate the need for and manage parenteral and enteral nutrition, including identifying specific nutrient requirements.
Educate patients and caregivers on customized tube feeding regimens.
Monitor and assess the effectiveness of nutritional care plans, recommending adjustments as needed.
Qualifications:
Bachelor’s degree in nursing from an accredited institution as approved and accepted by the University of South Alabama , three years of professional nursing experience, and licensure with the state of Alabama as a registered nurse; OR a bachelor’s degree with major coursework in human nutrition, food and nutrition, dietetics, or food systems management from an accredited institution as approved and accepted by the University of South Alabama, three years of professional dietitian experience, registry with the Commission on Dietetic Registration and licensure with the state of Alabama as a dietitian. Certified Diabetes Educator is preferred. MSN Highly preferred. Experience in diabetes care and chronic disease program management is preferred
