An exciting opportunity has arisen to bring your skills and expertise to help improve Diabetes Management for all people living with Diabetes in the Tameside and Glossop area. Do you have a passion for Diabetes, interested in working within an integrated Diabetes Service? Tameside & Glossop Integrated Care NHS Foundation Trust are currently recruiting for a Diabetes Specialist Nurses band 6 to join the Integrated Diabetes Service, operating within the Tameside and Glossop area. The successful candidates will be working in a multi-professional service alongside Diabetes Specialist Nurses, Consultant Diabetologist, Diabetes Specialist Dietitian, Diabetes Specialist Podiatrists and support staff
The post holder will be required to support developing, the inpatient and community provision for diabetes care as the initial focus areas. However in time the opportunity to work with a range of other patient’s groups. Including Ante-natal clinic, community clinics, specialized clinics (i.e. Glucose sensors and insulin pump clinic and young persons’ clinic) will be provided. The role also includes Triaging referrals into the service, Telephone consultation. The post holder will also be expected to deliver on structured education programmes for patients and HCP’s. We are looking for a self-motivated and dynamic individual with a minimum of 3 years’ experience of managing Diabetes as a primary condition, have a sound clinical knowledge of Diabetes management and related therapies. To have completed a level 5 course in the management of Diabetes or happy to work towards The clinical specialist will work as an autonomous practitioner to provide a high quality, efficient and effective clinical service, delivering high standard of assessment, diagnosis, treatment and highly specialist expert advice to patients with Diabetes and their carer’s in accordance with the levels of the competences as defined in the KSF. In addition they must be able to communicate effectively, work well within a team environment. Have good organisation skills and the ability to prioritize workload are essential.
