To plan and prioritise patient caseload and implement treatment of patients in their own homes/care homes, carrying out consultations for patients who need oral nutritional support or enteral tube feeding, communication or swallowing/feeding support.
Supervision from Dietitians/SLTs is available for reference and guidance. As The post holder enables, supports and assists patients who are receiving individual treatment programmes which have been agreed with and planned by the Dietitians and Speech & Language Therapists - there is a huge risk of hospitalisation and a restriction to independence
This is an exciting opportunity for a motivated and innovative individual to join the Integrated Community Cluster Network team as a Band 4 SLT/ Dietetic Therapy Technician.
The Network Team consists of staff from District Nursing; Social Work; Occupational Therapy; Physiotherapy; Community Psychiatric Nursing; Pharmacy and has links of the third sector as well as Speech & Language Therapy and Dietetics. This holistic approach provides a full offer of support for adults with changing needs in their localities as well as meeting complexity of needs. Many are living with cognitive difficulties and some diagnosed with dementia. Acting within their professional boundaries, the post holder will provide support to the person they are working with and those important to them through holistic person centred practice. They will be a key member in providing an effective team around the person.
The post holder will be highly motivated with excellent organisational skills. You will work directly with the SLT and Dietitian in the team, providing support with swallowing difficulties, communication and malnutrition in the community.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
