What You'll Do This position is internally titled Coordinator, Dietetics. The Dietetics Coordinator supports Strive Health’s mission by proactively reaching out to patients via phone. This role focuses on patient engagement, providing generalized nutrition education, and encouraging individuals with chronic kidney disease and related conditions to re-connect with their care team. The technician works under the supervision of a Registered Dietitian to ensure accurate, supportive, and patient-centered interactions. Essential Functions Conduct outbound calls to re-engage with Strive Health services, including scheduling visits. Share general nutrition and lifestyle education related to kidney health, blood pressure, and blood sugar management (as guided by Strive Health educational materials). Answer basic nutrition questions within scope; escalate complex questions to a Registered Dietitian. Document patient interactions, education provided, and follow-up needs in the electronic health record. Identify barriers to engagement (e.g., transportation, food insecurity) and provide resources and document interactions. Maintain confidentiality, professional boundaries, and compliance with HIPAA standards. Qualifications Minimum Qualifications: Associate’s degree in Nutrition, Dietetics. Nutrition and Dietetics Technician, Registered (NDTR) credentialed. Experience with electronic documentation systems. Internet Connectivity - Min Speeds: 3.8Mbps/3.0Mbps (up/down): Latency
