Job Description: • Fitzroy location • Challenging and rewarding work environment • Part time (28 hours per week) Maternity Leave position until 19/1/27 About the Role You will work as a Care Coordinator to provide holistic care coordination to patients with complex needs to improve their health outcomes and prevent avoidable hospital presentations. As a Dietitian you will provide discipline interventions to support the health, wellbeing and self-management of any client accessing HIP services. This role is based across the Emergency Department and via outreach into the community to vulnerable patients. Early identification of ALERT patients in the Emergency Department. To engage with and provide outreach care co-ordination services for identified or referred clients of the Emergency Department post discharge, including undertaking comprehensive assessment and collaborative care planning to guide interventions Provide targeted dietary assessment, intervention, lifestyle coaching and clinical management of dietary related conditions and health behaviours. Classifications for this position will range from AJ1 - AJ4 ($94,478.80 - $106,906.80 per annum pro rata). Your Contribution Bachelor of Nutrition Science Minimum 5 years experience in relevant acute/community setting Eligible for registration under the Accredited Practicing Dietitians.
