• Provide personalized nutrition assessments and develop tailored care plans for patients with various medical conditions.
• Educate and counsel patients and their families on healthy eating habits, disease management, and lifestyle changes.
• Monitor patient progress, adjust plans as needed, and document all care provided accurately.
• Collaborate closely with physicians, nurses, and other healthcare professionals to ensure comprehensive care.
