Will consider any of the following for this position: Dietitian - Outpatient - provides clinical nutrition assessments, recommendations, and education to hospital and/or outpatients, including group and individual self-management diabetes education, depending on work location. May also, provide nutritional care to oncology, homecare and hospice departments. May be required to travel to other facilities. Diabetes Educator, Nurse provides group and individual self-management diabetes education to outpatients. Functions with a competency level of Diabetes Educator Level 1 to a Level 2. This position may be required to travel to other facilities. If a Dietitian, also provides clinical nutrition assessments and recommendations. Diabetes Educator, Nurse CDCES provides group and individual self-management diabetes education to outpatients. Functions with a competency level of Diabetes Educator Level 2, working towards a Level 3. This position may be required to travel to other facilities. Work Schedule: .7 FTE; 28 hours/week; weekdays Required Qualifications: Dietitian – Outpatient: (pay range $28.99 - $43.49) Degree in Dietetics Experience with demonstrated skills in all phases of Diet Therapy and Diabetics self-management education If working in outpatient setting, must obtain Certified Diabetes Care & Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education (CBDCE) once requirements for testing are met Must be Registered Dietitian through Commission on Dietetic Registration (CDR) upon start Must be licensed as a Certified Dietitian through WI Department of Safety & Professional Services (If WI certification is not present at hire, must obtain within 4 months of start) Current BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider) Diabetes Educator: (pay range $35.66-$53.49) Degree in Nursing or Dietetics If a nurse, must obtain Certified Diabetes Care & Education Specialist (CDCES) through the Certification Board for Diabetes Care and Education (CBDCE) once requirements for testing are met If Registered Dietitian, must be Registered Dietitian through Commission on Dietetic Registration (CDR) upon start If Registered Dietitian, must be licensed as a Certified Dietitian through WI Department of Safety & Professional Services (If WI certification is not present at hire, must obtain within 4 months of start) If a Dietitian, must be Certified Diabetes Care & Education Specialist (CDCES) through Certification Board for Diabetes Care & Education (CBDCE) If Registered Nurse, must be licensed in both MN and WI upon start date.
