Provides nutritional counseling/support to patients upon physical referral.

QUALIFICATIONS:

• Registered Dietitian (R.D) and Certified Diabetes Educator (C.D.E.) required.
• B.S. in Nutrition with accredited dietetic internship, preferred.
• Two to four years related experience and/or training, preferred.
• Equivalent combination of education and experience.LICENSURE, CERTIFICATIONS, AND CLEARANCES:

CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

• Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
• Registered Dietitian (RD)

• Reviews and applies dietary orders written by the physician.
• Prescribes dietary modifications at the physician’s request to make it easier for the patients to follow their dietary regimen. Modifications are documented in the patient’s chart.
• Develops individualized meal plans for patients to help them stay within their prescribed dietary orders. Appropriate documentation is noted in patient’s chart.
• Interviews patients to obtain diet histories and assess nutritional status.
• Provides nutritional advice to physicians upon request to help them better understand what dietary regimen would be most helpful to the patient.
• Educated/counsels patients regarding their nutritional needs as it relates to their diagnosis or problem. Appropriate documentation is noted in patient’s chart
