Description · Work on service plan goals with the member as outlined in the service plan. SR · Provide skill development to members in their home and community as outlined in the service plan. Utilize intervention steps and document member response and engagement, as well as staff intervention in EHR. SR, SL, NV, D, OC · Identify and utilize the member’s safety plan and self-care plan when in crisis. Report to Case Manager. SR, OC, SL, NV · Provide transportation to members adhering to goal completion. SR · Acquire appropriate signed documentation from members as assigned by the Case Manager or Program Director. SR · Complete a SAFE-T with members when presenting or reporting suicidal ideation and communicate with Case Manager and/or Program Director. SR, NV, OC, EI · Work with the Case Manager and team to brainstorm skill-building activities. SL, D · Track member goal completion and report to the Case Manager. SR · Track member behaviors and trends. Report to Case Manager. SR · Complete service documentation within 72 hours of service being provided. SR · Attend weekly team meeting and scheduled supervision. SR, SL, OC, EI, GC, D · Attend job-specific trainings as assigned and necessary to provide services. SL, GC · Complete member surveys with members. SR, D, OC · Maintain a valid driver’s license. SR · Complete Sanctuary Training within 90 days of hire and utilize tools during services (i.e., community meeting, SELF, safety plans, etc.) SR, SL, EI, OC, GC, NV, D · Abide by The Health Insurance Portability and Accountability Act of 1996 (42 USC & 201 et seq.) SR, OC · All other duties as assigned.
