EDUCATION AND QUALIFICATIONS:

  • A high school diploma or equivalent is required.
  • A bachelor’s degree in a human service field such as sociology, social work, mental health, specialist education, rehabilitation counseling, nursing, or psychology is preferred.
  • Any combination of equivalent experience and/or education as approved.
  • Ability to maintain a valid driver’s license, availability of an automobile and abilities to meet the criteria as set forth by the organization’s automobile insurance company.
  • Passing score on the Medicaid waiver exam as designated by DBHDS.
  • Maintains current CPR/first aid certifications and crisis prevention intervention trainings.

REQUIRED INFORMATION TECHNOLOGY (IT) SYSTEMS SKILLS AND PROFICIENCY:

Beacon Specialized Living Services (BSLS) uses a variety of information technology information systems including Microsoft Office 365 including Word, Excel, Power Point, Outlook, Teams, and OneDrive. You will be responsible for learning and becoming proficient with these IT systems and others that BSLS may deploy or change in the future.

Working Environment:  
Most of the job duties will be performed at a home-like setting or community facility with the potential of exposure to infectious disease. Universal precautions are required to minimize the risk of infections. Work sites may include locations that are not barrier-free. Duties are performed in an environment where there is potential for physical aggression from individuals.

Responsibilities

PRIMARY RESPONSIBILITIES:

  • Provides support as necessary to ensure needs as identified by the individual and his/her inter-disciplinary team are met.
  • Communicates and coordinates with families, members of the inter-disciplinary team (IDT), the individual’s day program and other providers of services and offers support as necessary and appropriate.
  • Instructs, supports, and participates in performing household tasks with individuals such as cleaning, preparing meals, planning household budgets, food shopping, etc.
  • Assists individuals to develop daily living skills by identifying and meeting social, environmental and health care needs through person-centered planning and self-determination.
  • Monitors individuals to ensure they take medication as prescribed and teaches them how to take medication independently, may assist with medication administration when out in the community with the individual at a time when medication is to be taken.
  • Performs crisis intervention; provides emergency counseling and assistance.
  • Plan, coordinates and participates with individuals in person-centered community inclusion and leisure activities.
  • Participates in assessment activities and on the IDT as appropriate.
  • Transports individuals and/or provides mobility training to assist individuals to go to appointments, community rehabilitation program, grocery shopping, etc.
  • Communicates and documents activities, progress, and behaviors of individuals as appropriate.
  • Prepare food according to program developed menu, individuals’ likes and dislikes and doctor recommended diet.
  • Attends staff meetings, training in-services, and house and program meetings as directed.
  • Ensures program compliance with Beacon policies and DBHDS regulations.
  • Inspects the home and reports repairs and maintenance needed.
  • Ensures fire and other safety procedures area carried out as directed.
  • Maintains proper sanitation and security conditions within the residence.
  • Monitors activities of visitors to the home.
  • Has up-to-date knowledge of Beacon policies and procedures.
  • Provides direct care support to individuals supported by the program including transportation and applicable lifting requirements.
  • Maintains knowledge of and adheres to Beacon’s Mission, vision, and core values.
  • Works a flexible schedule that may change or vary over time according to the needs of the program.
  • Performs and/or assigns other related duties.
