Overview The Personal Assistance Services and Supports (PASS) worker helps families of children with special health care needs with daily life skills. The direct support provided through PASS aims to improve a child’s ability to participate in social situations and function in the community with dignity. This position works with families and their children to develop home and community-based support systems with the goal of building self-determination and independence. PASS maximizes opportunities for each family to help their child succeed in the least restrictive and most natural setting. Seven Hills has opportunities for direct care providers with many years of experience, as well as for college students, stay-at-home moms and dads, paraprofessionals, caregivers, DSP, Residential Counselors or teachers looking for a part time employment. Responsibilities Implements direct personal assistance goals developed by the family in conjunction with agency with the individual.Provide safe one to one care per the plan for children with special health care needs as directed by parents/caretakers.Provide socialization opportunities within home and/or outside of home when appropriate.Perform all necessary self-care tasks as needed by the individual supported.Documents progress on required data sheets and progress notes which are made available to family and clinical consultantAssists people with learning about and advocating for their personal rights and understanding the responsibilities that accompany those rights.Drives people to destinations, as approved by parent/caretaker necessary for them to experience a full and diversified life, following all safety and legal regulations.May also participate in a program participant’s chosen activity Qualifications Required Qualifications: 18+ years old High School Diploma or equivalent Valid Driver’s License & Good Driving Record Preferred Qualifications: Commitment to assisting people with varying disabilities to participate fully in their community and in their own lives. Varied interests and personal connections in the local community. Proven record of conscientious, responsible behavior (work, volunteer or school history). Ability to work as a creative, engaged, dedicated member of your team.
