Description Job Description Participate as an active member of the interdisciplinary team by implementing the Individualized Support Plan and providing ongoing feedback regarding the individual's progress. Serve as an advocate for the individual, ensuring that all health and safety requirements are addressed with respect and dignity. Job Duties and Responsibilities 1. Assist in assessing the individual’s progress and provide input to the team regarding the development of appropriate plans to achieve designated ISP objectives. 2. Complete daily documentation of progress through Therap, including written and verbal reports as appropriate. 3. Serve as a positive role model to individuals served and colleagues. 4. Support the development of life skills and implement behavior management strategies. 5. Assist with personal hygiene, grooming, medication administration, household chores, laundry, meal preparation, and other activities outlined in the ISP. 6. Provide transportation to community activities using personal vehicles, as needed. 7. Identify and facilitate access to community resources, including social, volunteer, recreational, educational, and financial services, in addition to scheduled activities. 8. Offer emotional support and constructive feedback to individuals and their families. 9. Complete all required training and adhere to HeartWell policies and procedures. Required Skills Patience, compassion, and strong interpersonal communication Ability to handle challenging behaviors calmly and professionally Dependability and time management Basic household skills (cleaning, cooking, laundry) Requirements Position Requirements Minimum -High school diploma or GED Must be at least 18 years old Valid driver’s license with reliable transportation Motor vehicle Record Safe insured/ registered vehicle Ability to pass a background check and drug screening Previous caregiving experience preferred, but not required
