Roles and Responsibilities:

  • Provide strategic and operational leadership to deliver exceptional, patient-centered care aligned with HFHT’s mission, vision, and strategic priorities.
  • Develop, implement, and evaluate operational plans that prioritize the patient and provider experience while ensuring compliance with policies, procedures, and standards.
  • Collaborate with HFHT leaders, physician members, practice staff, external partners, and community stakeholders to advance population health, place-based care, and integrated service delivery.
  • Oversee the standardization and optimization of clinical and operational workflows across sites to improve efficiency, consistency, and equity of care.
  • Monitor and analyze quality, performance, and resource utilization, identifying and addressing gaps in care delivery.
  • Champion initiatives that enhance patient experience, address regulatory and emerging issues, and respond to feedback, incidents, and complaints constructively.
  • Allocate resources effectively to achieve operational and strategic goals while ensuring fiscal responsibility and mitigating risks.
  • Lead and mentor multidisciplinary teams through organizational change, cultivating a high-performance, psychologically safe, and inclusive workplace.
  • Develop succession plans and leadership capacity within teams to ensure organizational sustainability and resilience.
  • Represent HFHT on multidisciplinary committees, regional planning tables, and in professional forums.
