Learn more at: www.navvishealthcare.com Role Overview: The Director of Clinical Transformation serves as a critical component for managing the TEAM model with our partner Hackensack Memorial Health in New Jersey. This Director will oversee the episode performance for the 9 hospitals of the HMH system that will participate in TEAM. This individual will be the in-market representative expert that regularly engages with stakeholders to produce continued performance and ensure clinical and financial success. The Director of Clinical Transformation will oversee a team of Episode Coordinators to ensure optimized patient outcomes. As a Director of Clinical Transformation you will: Lead the Hackensack Meridian Health TEAM episodes to ensure success in performance Facilitate co-design of episode pathways with Navvis physician support Support adherence to workflows and refine pathways as indicated Review clinical and operational reports to identify gaps in performance and develop strategies to address Manage Care Episode Coordinators through management of caseload and success Facilitate hospital and system report outs to ensure awareness of TEAM performance Serve as an In-market subject matter expert on TEAM to system stakeholders Prepare high quality and compelling presentations for delivery to clients and participate in subsequent action planning related to gaps and opportunities Coordinators being managed will cover these locations: o Hackensack University Medical Center o Palisades Medical Center (Safety Net) o JFK University Medical Center o Raritan Bay Medical Center o Jersey Shore University Medical Center o Bayshore Medical Center o Riverview Medical Center o Ocean University Medical Center o Southern Ocean Medical Center What Success Looks Like in this Role: High degree of initiative, professionalism, judgement, and discretion Ability to be seen as a strong influencer/motivator and credible client resource Ability to develop organized and appropriate prioritized work Requirements We are excited about you if you have these things: RN or NP required. 5-7 years working to improve Care Episode performance for a Health System, Payer, or Physician Organization in these areas: BPCIA and CJR Models Advanced Alternative Payment Models such as MSSP, NGACO, ACO Reach, Direct Contracting Medicare Advantage Health Plans Population-based Improvement Strategies Superior writing and presentation skills Strong critical thinking, problem identification, resolution skills What you'll get: Navvis is committed to attracting the most insightful and motivated talent by providing a candidate and onboarding experience that you won't find elsewhere!
