DIRECTOR OF CARE

Fort Amherst Healthcare campuses are dedicated to providing safe and supportive living options for aging individuals. The Director of Care is fundamental in care plan development and provision of care at their designated location. The successful candidate will work closely with the General Manager to ensure a person-centric approach is employed, facilitating safe and effective care for residents.
The Director of Care is passionate about clinical standards and expectations and will help to develop and implement policies and procedures to ensure the facility’s clinical standards continuously meet the Regional Health Authority and Accreditation Canada guidelines.
The Director of Care is committed to building strong and trusting relationships with residents, family members, members of the RHA monitoring team, and other members of the multidisciplinary team by fostering an environment of openness, respect, and clinical excellence.

Duties and Responsibilities

  • Hiring and orientation of care staff including Personal Care Attendants and Unlicensed Care Providers (Medication Administration)
  • Supervising, mentoring and coaching care staff by facilitating ongoing training and education and conducting performance audits and reviews.
  • With the Administrator, ensures all employment documentation received and filed appropriately.
  • Completes departmental scheduling within defined labor budget, ensuring that operational standards are met.
  • Submits completed the departmental schedule to the General Manager for approval and posting, meeting all set deadlines.
  • Oversight of departmental supply budget.
  • On-Call responsibility for facility management and staffing support on rotating schedule in conjunction with other members of the management team.
  • Provides emergency coverage to meet staffing standards as required, ensuring operational standards are met.
  • Ensures all members of the care team complete monthly training, as assigned.
  • Works with General Manager to facilitate new resident admissions by:
  • Reviewing new referrals from Regional Health Authority
  • Creating Resident Care Plan, updating and delegating care team.
  • Completes all required pharmacy paperwork, ensures all clinical guidelines are met on day of admission.
  • Completes MCP change of address form.
  • Completes Resident Orientation Checklist, ensuring resident and family are orientated to the building.
  • Coordinates with physicians, families, and other members of the multidisciplinary team to ensure care plan is appropriate and care needs are met.
  • Updates care assignment sheets and ensures appropriate delegation of care tasks.
  • Participates in quarterly auditing process with RHA monitoring team.
  • Completes internal auditing, as scheduled.
  • Ensures all Operational Standards are met.
  • Ensures resident satisfaction and continuous quality improvement initiatives are in place.
  • Ensures that resident care plans are continuously updated and reflect current care needs.
  • Communicates any change in condition or care needs to RHA monitoring team.
  • Monitors the quality of care being provided, implements long-term strategic plans.
  • Actively participates in physicians’ rounds.
  • Ensures accurate processing of new medical orders.
  • Prepares and submits interdisciplinary referrals, as needed.
  • Coordinates client appointments, as needed.
  • Ensures accurate documentation is complete.
  • Ensures that staff are trained and complete accurate documentation including incident reports and progress notes.
  • Reviews incident reports daily, implements required changes to care plans, action plans, etc. to reduce risk.
  • Maintains open communication with RHA monitoring team.
  • Reviews progress notes daily.
  • Provides General Manager will be required information to complete month-end reporting.
  • Updates resident care plans quarterly and as needed to reflect changing care needs.
  • Submits weekly auditing promptly, meeting given deadlines.
  • Clinical Auditing functions:
  • MAR Check – ensures accuracy in documentation (signatures, PRN’s, etc.)
  • Med Cart Audits minimum once weekly.
  • Completes Medication pass audit with staff at minimum once per quarter.
  • Ensures all required staff training meets RHA standard (Medication Policy and Procedure)

Job Types: Full-time, Permanent
Pay: $65,000.00-$68,000.00 per year
