Description Assure that initial and ongoing assessments are completed and submitted as necessary to assist plan development, to maintain service eligibility, and as required by various regulatory bodies. Assure that the health care needs of persons supported are addressed, including scheduling physical and mental examinations and other health assessments on a regular and as needed basis, attending such appointments as necessary. Assure that all health care and other assessment results and recommendations are followed and any necessary follow up conducted within allowable time frames. Assure familiarity with the people supported, including being aware of their vision for the future, their needs and desires, the specific supports that they receive. 5. Attend and actively participate in circle of support meetings on behalf of persons supported by the team as needed. Maintain relationships, open communication, and cooperation with the individuals supported, their families, conservators/guardians, or other representatives. Manage the Health Coordinators and assist them as needed in their job duties. Assure that people supported receive all TennCare Certifications. Assure that all records, reports, compliance grid and other documentation relating to people supported or otherwise relevant to team activities are maintained in an orderly, current, and secure fashion. Establish and implement procedures to ensure an effective flow of communication among all team members, including holding regular meetings with team members and LPN’s on at least an annual basis. Assures planning, monitoring and appraisal of employee work results; hearing and resolving conflicts; counseling employees as needed. Actively participates in Reportable Events Management. Maintain communication with team members on regular basis by carrying and responding electronic messages and telephone calls in a timely fashion. Maintain communication with team needs by periodically carrying and responding to the team emergency telephone, including addressing staff call outs, staff queries, and incidents requiring intervention Assures employees have adequate skills to perform their essential job duties by assessing their training and staff development needs; providing or securing necessary training or retraining; focusing on best practice approaches; conducting periodic informational meetings. Provide direct support for persons supported by the team when necessary or when other qualified staff are not available. Assisting with end of life required reporting and communications with all responsible parties. Assure that the rights of people supported are protected, including healthcare rights. Assure that people are supported in a manner which enhances their dignity and respect. Assure confidentiality of all privileged information and records concerning persons supported, their families, team members, or other agency staff, as well as any additional information concerning the agency, its staff, clientele, or operations which may be considered to be of a confidential nature. Actively participate accreditations, licenses, and required surveys. Assure that the mission of the agency, the goals and objectives of the team, and the outcomes for people supported are addressed and realized to the greatest extent possible. Coordinate emergency placements with hospitals, mental health hospitals, respite agencies, hospice, long term care, etc. Assisting with conservatorships, POA, healthcare advocate, and other legal paperwork as needed. Coordinate with contracted healthcare services. Addresses family, case managers, and conservator complaints in a timely manner. Requirements Bachelor’s degree in one of the behavioral sciences, and two years’ experience in developmental disabilities or related field, or any combination of college and experience with one year of experience equating to one year of college. Must possess demonstrated leadership ability, the ability to communicate effectively with diverse audiences both orally and in writing, a sensitivity to the needs of other people. Must possess valid Tennessee driver’s license and be insurable under agency insurance programs.
Responsibilities
The Director of Health & Social Services is responsible for ensuring that health care needs are met for individuals supported, including assessments and follow-ups. They also manage Health Coordinators and maintain communication with team members and families.
