Job Details Level: Experienced Job Location: Dilkon Medical Center - Dilkon, AZ 86047 Position Type: Full-Time Education Level: Bachelors Degree Travel Percentage: Occasionally Job Shift: Any Under the general supervision of the Chief Community Health Officer, the Director of Hozhoogo Iina Wellness Program will provide full responsibility for supervision, planning, budgeting, development, organization, integration, and evaluation of the programs under the Hozhoogo Iina Wellness Program. Oversees the clinical, outreach, and home visit services under the Hozhoogo Iina Wellness Program. Serves in the acting capacity as the Chief Community Health Service as needed. Assists the Director of Community Health Services and managing various grants under the community health service division. Upholds the principles of WIHCC’s Vision, Mission and Value Statements. Maintains confidentiality of all privileged information at all times. This list of duties and responsibilities is illustrative only of the tasks performed by this position and is not all-inclusive. Essential Duties & Responsibilities: Maintains regular attendance and punctuality. Supervision of community health service personnel, which includes work allocation, schedules, time approval, hiring, training, orientation, and problem resolution; evaluates performance and makes recommendations for personnel actions; motivates employees to achieve peak productivity and performance. Develops measurable performance expectations for assigned staff according to department goals and objectives. Oversees the daily administrative operations for the designated health services. Ensures clinical and service quality in collaboration with the PCMH teams and community health services programs. Develops and maintains annual department budget and the departmental strategic goals. Serves as a liaison for WIHCC’s community resources. Works with key members of the management team, department managers, and supervisors; fully participates in management, strategic planning and policy decisions and shares accordingly in implementing plans. Participates with the formulation of policies and procedures related to the target population’s health needs with an emphasis on secondary and tertiary levels of prevention across the age spectrum. Develops and implements plans for expansion or revision of present programs and services to meet the changing needs of the target population. Serves as a member of the WIHCC area wide committee and task forces as approved by the Director of Community Health Services. Works with the Director of Community Health Services to promote an understanding of the population health programs with a focus on disease prevention, treatment. Participates in planning, implementing, and evaluating the population health programs and policies to meet the WIHCC strategic plan in conjunction with the organization’s mission, vision, and value. Ensures Evidence-Base standards of care are implemented and compliant with all state and federal regulations, standards of practice, and WIHCC’s policies and procedures. Develops and maintains policies and procedures to assure compliance with regulatory standards of Joint Commission and Centers for Medicare and Medicaid Services (CMS); to include American Association of Diabetes Educators (AADE). Plans and prepares for periodic accreditation review. Resolves critical problems which may arise within the DM clinical and community health service line. Works with projects to a timely resolution and completes assignments according to agreed deadlines. Works diligently with other departments on various projects as needed or assigned and updates the appropriate persons of the progress of the project(s) in a timely manner. Develops, manages, evaluates and implements quality management plan to improve quality, access and effectiveness of the target population’s health services and further the WIHCC’s service area strategic plan. Provides oversight and direction for on-going, systematic evaluation of population health services with a focus on health prevention and disease prevention using established methods and nationally recognized standards; proposes and implements performance improvement projects to address identified issues. Aligns the organizational mission, vision, and value with the division of community health service strategies, activities, and interventions with a focus on the WIHCC strategic goals. Monitors the community health needs and priorities of the target population using data on demographics, health care utilization-the RPMS generated reports, health problems, risk behaviors and community resources; assists communities with identifying and interpreting health information and data. Assists the Director of Community Health Services in providing direction and coordination of grant applications and management of awarded funds under the community division to assure funding requirements, goals and objectives are met to meet program and community needs. Works closely with clinical consultants to the program to provide guidance in the coordination, planning, implementation and evaluation of prevention community health service programs, such as screening programs for youth and adults. Provides recommendations on program improvements to the appropriate program staff, departments, and executive team. Works with the Director of Community Health Services to provide recommendations and oversight for the design, establishment, evaluation, and utilization of database systems to enhance surveillance, evaluation, tracking and monitoring the health status of the target population within the WIHCC service area. Works with the Director of Community Health Services to maximize the use of alternative resources through collaborative relationships with universities, schools, community and non-profit groups and tribal, county, state and federal governmental agencies. Works closely with Division staff to identify outside funding sources for program development and sustainability and for research into innovative solutions to health problems. Serves as consultant for other WIHCC staff, including clinic nursing, community health nursing, and other disciplines regarding grants management. Requires completion of tasks or duties assigned by a supervisor. Upholds all principles of confidentiality and patient care to the fullest extent. Adheres to all professional and ethical behavior standards of the healthcare industry. Possesses cultural awareness and sensitivity. Qualifications Minimum Qualifications: A Master’s Degree in Administration, Health Administration, Public Health or clinical domain; or bachelor’s degree in Health related field and license as a Registered Nurse required. Five years of population health experience plus five years management experience. Community Health or Health Promotion experience preferred. Bilingual skills in English and Navajo language preferred. Must maintain a valid unrestricted and insurable driver’s license. Must successfully pass a background investigation and maintain suitable requirements for a Child Care position. This position is considered as a Child Care position, which requires a satisfactory background check investigation and is subject to the requirements of the Indian Child Protection and Family Violence Prevention Act, as amended (henceforth referred as the ICPFVP Act). Knowledge of statistical software (e.g., SAS, SPSS, Epi-Info) and other computer software (e.g., Microsoft Word, PowerPoint, Excel, ACCESS) used analyze and present health data. Knowledge of educational systems, public health legislation, policies and resources, and public health services at the federal, state, tribal and local level to optimize program planning and coordination and delivery and access to public health services. Knowledge of the WIHCC Medical Staff Bylaws, rules and regulations and all requirements for appointment to and maintain status as an active member of the medical staff; Knowledge of grant application and management including Federal funding requirements to manage, oversee, and/or monitor program or project performance and funds distributed through grants, cooperative agreements, contracts, and other awards. Knowledge of and ability to apply administrative and managerial concepts for the purpose of planning, developing, conducting and evaluating diabetes care. Knowledge of pertinent programs and departments within WIHCC, Navajo Area Indian Health Service, and tribal and community-based organizations involved in diabetes prevention and treatment efforts.
