Position Summary: The Director of ISP Planning & Outcomes Management is responsible for overseeing the end-to-end system for ISP goal development—including workflow structure, goal creation, Therap entry, ongoing tracking, and outcome measurement. This position ensures the quality, consistency, and integrity of the entire ISP process. The Director establishes a standardized, person-centered system that produces meaningful, measurable goals and reliable documentation across all programs. What We Need: 1. Goal Development Process Oversight (Structure and Workflow) Design and manage the standardized agency-wide process for developing ISP goals. Establish clear workflows for assessments, interdisciplinary contributions, family/guardian input, and final goal creation. Ensure goals are measurable, aligned with assessments, and consistent. Train and guide key staff to follow the structured system for goal development. 2. Goal Creation and Therap Implementation Oversee the consistent creation of ISP goals and translation of those goals into Therap ISP Programs. Maintain standardized formats, templates, naming conventions, and system rules within Therap. Ensure Therap entries are accurate, complete, and aligned with the structured development process. Serve as the organizational subject-matter expert for ISP components in Therap. 3. Goal Tracking System Define the structure, frequency, and expectations for goal tracking across programs. Ensure staff understand and follow the standardized method of documenting progress in Therap. Build processes that allow for consistent monitoring of progress without directly performing audits. 4. Goal Outcome and Achievement Framework Develop the agency’s framework for defining, measuring, and reporting goal outcomes. Ensure that programs understand how to determine whether goals are met, partially achieved, or require revision. Establish reporting expectations for teams to summarize progress at quarterly and annual intervals. Support selection and design of goals that lead to meaningful outcomes that support progression within programs. 5. System Integrity and Quality of the Process Ensure the processes, workflows, templates, and staff practices produce consistent, reliable ISP documentation. Maintain the integrity of consumer planning data by ensuring uniform practices, language, and documentation standards. Serve as the steward of the ISP process, ensuring the system remains compliant and efficient. What You Need: Strong interpersonal, organizational, and writing skills. Strong knowledge and expertise in Therap. Ability to define problems, establish facts, and draw valid conclusions. Valid driver's license with no more than 3 suspensions and/or moving violations in the past 3 years. Why Join Us?
