Job Details Job Location: Dwight, IL 60420 Position Type: Full Time Salary Range: $70,000.00 - $73,000.00 Salary Responsibilities: LPN's are welcome to apply. Conducts initial assessment of prospective residents, including off-site visits Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences Coordinates the Community’s infection control program while maintaining state regulatory compliance Develop and oversee the Community’s medication management program Contributes toward quality assurance and performance improvement initiatives Complies with all rules and regulations regarding confidentiality and privacy of resident information Qualifications Qualifications: Maintains current state nursing licensure.
