Director of Patient Care Services (DPCS) - Home Health Location: Illinois & Indiana | Full-Time | Clinical Leadership About the Role One Home Health is seeking an experienced home health nurse leader to serve as our Director of Patient Care Services (DPCS). In this role, you'll lead clinical operations across our Illinois and Indiana service areas, ensuring excellent patient outcomes, survey-ready compliance, and a high-performing clinical team. What You'll Do Direct and supervise all clinical services across both service areas, with periodic field presence to coach and support staff Ensure care is delivered per physician orders, the plan of care, CMS Conditions of Participation, and state requirements Lead the QAPI program; tracking hospitalizations/OASIS outcomes/HHVBP metrics Maintain survey readiness and lead responses to surveys, audits, and plans of correction Oversee staffing, competency, documentation, OASIS accuracy, infection control, and emergency preparedness Partner with intake, sales, and operations to support appropriate admissions and growth What You Bring Active Registered Nurse (RN) license with Illinois required; Indiana strongly preferred (dual licensure preferred for the full scope of the role) Minimum 2 years of clinical experience, with at least 1 year in home health Working knowledge of CMS Conditions of Participation (42 CFR Part 484), OASIS, care planning, and Medicare regulations Experience in clinical supervision and staff management; prior DON/DPCS or Clinical Manager experience preferred Survey readiness and regulatory compliance experience, ideally in a multi-state agency Why One Home Health Join an agency investing in its clinical leadership and building for the future.
