THE ROLE As our Geriatric Resource Specialist, you will be the trusted expert guiding residents and families through life’s transitions with grace, clarity, and world-class service. This role is intentionally crafted for a licensed social worker who thrives at the intersection of advocacy, hospitality, and relationship-building. You will: Conduct thoughtful psychosocial assessments that honor each person’s story, strengths, and goals. Lead lifestyle care coordination with consistency, follow-through, and Ritz-level attention to detail. Build and maintain a strong network of Atlanta partners — hospice, home care, hospitals, discharge planners, wellness providers, and community programs. Support residents and families through referrals, transitions, care changes, and problem-solving moments. Facilitate support groups and educational programs that build trust and belonging. Collaborate seamlessly with Sales & Marketing, Lifestyles, and Risk/Clinical partners to ensure clear communication and exceptional resident experience. Travel between three Holbrook campuses as the expert resource — working independently with executive-level professionalism. Maintain documentation excellence, budget stewardship, and the unwavering advocacy our families rely on. Occasionally support our Assisted Living neighborhoods and family partners. This is a role for someone who is energized by people, passionate about aging well, and committed to an elevated standard of service. Requirements WHAT YOU BRING Georgia Social Work License — LMSW strongly preferred. 3–5+ years of experience supporting older adults in senior living, hospital discharge planning, home care, or related environments. Exceptional communication and follow-up — the kind that makes people feel seen, heard, and supported. A preventive, solutions-driven mindset rooted in both care and accountability. Proven ability to manage large caseloads with poise, clarity, and organization. Deep Atlanta connections — you know the local network because you’ve built it.
