Description Vineyard Hills Health Center, 99-bed skilled nursing facility, is now hiring for Social Services Director! We are looking for someone who has compassion and wants to help make a difference in the lives of our residents. This is a full-time position with 5 days a week. Social Service Director Job duties will include but is not limited to: • Interviews new residents, their family and/or responsible party to assess, document and develop plans to meet the resident’s psychosocial needs as appropriate. • Updates assessments and care plans at least every three months and as needed. • As the resident’s advocate, communicates with family, friends, other agents and community resources as appropriate to make referrals and obtain services necessary to assist residents to maintain or improve their ability to manage their everyday physical, mental and psychosocial needs in accordance with state and federal guidelines. • Works with resident, their representatives and appropriate facility staff to coordinate discharge planning and services. • May facilitate resident support group and/or resident council meetings per individual facility guidelines. • Assists with the resolution of resident or family requests, concerns or complaints as designated by Administrator. • Provides information about and helps secure resources to assist residents with completion of State Aid forms. • Keeps immediate supervisor promptly and fully informed of all problems or unusual matters of significance and takes prompt corrective action where necessary or suggests alternative courses of action which may be taken. • Performs all duties and responsibilities in a timely and efficient manner and in accordance with established company policies to achieve the overall objectives of this position. • Maintains a favorable working relationship with all other company employees to foster and promote a cooperative and harmonious working climate which will be conducive to maximum employee morale, productivity, and efficiency. • Reports residents' health concerns to the Nurse on duty and follows up with health and all other resident concerns at the morning IDT meeting. • Ensures residents' confidentiality. • Performs other duties and responsibilities as required or requested. Preferred Degree in Social Work or in any related field. 2. Previous training and experience in skilled nursing administration is preferred. 3. Has good verbal and written communication skills and demonstrates capability to work effectively with all employees. 4. Demonstrates interest and ability to work effectively with geriatric and disabled persons as well as their family members and/or responsible parties. 5. Has the ability to generate enthusiasm and cooperation from others and to develop and maintain good interpersonal relationships with physicians, discharge planners, social workers and other community contacts. 6. Has the ability to manage multiple tasks with frequent interruptions. 7. Has proficient computer skills.

Responsibilities

The Social Services Director interviews new residents and develops plans to meet their psychosocial needs. They also coordinate discharge planning and advocate for residents' needs with families and community resources.
