About the Role: Reporting to the Senior Director, Integrated Care, the Director of Strategic Initiatives is responsible for coordinating, driving, and delivering complex, whole-of-system strategic initiatives that improve integration across acute, primary, community, and social sectors in alignment with the Vision, Mission and Purpose of the Integrated Care Program: Vision: An integrated health system with the person at the centre of their care. Mission: Creating relationships and improving systems to drive seamless person-centred care. Purpose: Building capacity together to improve outcomes and experiences in support of the right care, at the right time, in the right place. You will support the operations, development, and continuous improvement of the Integrated Care Program including the Paediatric Liaison and Navigation Service (PLaNS), the Liaison and Navigation Service (LaNS), Priority Populations and other projects or service development initiatives. You will lead, coordinate, and contribute to the ongoing development of Integrated Care Program services and initiatives to ensure the evolution of sustainable and evidence-informed processes and programs to improve integration of care across the system. As a senior officer and active member of the Integrated Care Management Team, you will also play a key role in supporting the effective operational leadership of the service, including fostering positive culture, growth, capability, and wellbeing of individuals and teams. The position works closely with a wide range of stakeholders both within CHS and across the broader health and community sectors. To be successful in this role, you will demonstrate a high-level of professionalism, be confident managing high workload in a dynamic environment, be adaptable and responsive, be able to quickly acquire knowledge and understanding of situations and subject matter and be able to cultivate productive working relationships.
