The Director of Waiver Services Upstate Region is responsible for the daily operation of the TBI and NHTD Medicaid Waiver Programs serving the Buffalo, and Rochester Branches, and their local communities. The Director of Waiver Services Upstate Region must possess prior management experience, as well as strong knowledge of the Medicaid system. The Director of Waiver Services Upstate Region is responsible for the training of all waiver staff associated with the programs, quality assurance/ improvement, regulatory compliance with the NYS DOH, and overall business growth. The Director of Waiver Services Upstate Region is also responsible for overseeing the Waiver program including Case Management, Home Care, Skills Building, and Counseling, growing the Waiver Program.

Responsibilities
  • Oversee the caseload of all direct reports in all locations.
  • Achieve established growth parameters for each region to become the preferred provider of choice.
  • Review referrals for service coordination services, complete intake process, and ISP as appropriate.
  • Review, update, and submit service plans to the RRDS/RRDC for review.
  • Develop and periodically review Waiver Program policies and procedures with the Regional Director of Waiver Services.
  • Organize and facilitate team meetings.
  • Educate agency staff and community partners on Waiver Services and program requirements.
  • Organize monthly individual supervisory meetings with each Service Coordinator.
  • Maintain records for at least (7) seven years after termination of waiver services.
  • Ensure completion of a UAS at least every twelve months or when the participant experiences a significant improvement/ decline in his/her life ability to function independently in the community.
  • Ensure that all waiver service providers and the participant receive a current copy of the most recently approved service provider.
  • Maintain knowledge of all approved waiver services providers in their region.
  • Fill in when necessary for Service Coordinators’ in-home visits on an as-needed basis.
  • Complete and submit Serious Reportable Incidents (SRIs) as needed.
  • Conduct random audit visits with participants quarterly to verify leave of care.
  • Complete, at a minimum, one face-to-face visit annually for each participant supervised by our Service Coordinators.
  • Assist the prospective participant to become a waiver participant and develop, implement, and monitor the provision of all services in the service plan.
  • Ensure that the service plan is implemented appropriately.
  • Act as an effective advocate for Community Care’s Waiver program by building a network through relationships with providers and potential participants.
  • Ensure that the participant is receiving appropriate and adequate services from providers.
  • Provide supervision and support to the Waiver Service Coordinators.
  • Review and sign off on ISP’s, RSP’s and addendums.
  • Review, sign off on, and track Serious Reportable Incidents.
  • Supervise all Service Coordinators in the Upstate region.
  • Attend mandatory trainings /meetings conducted by the Regional Resource Development Center for the region(s) served.
  • Maintain positive work relationships both internal and external to the agency.
  • Perform any other duties as assigned by management.
