SKILLS AND EXPERIENCE

  • Minimum qualification at Certificates III or IV level in one of the following: Individual Support, Disability, Ageing Support/HACC, Mental Health, Mental Health Peer Work, Community Services or Allied Health Assistant OR at least 1 year of relevant work experience.
  • Current First Aid Certificate (including CPR)
  • Current Australian Drivers Licence or accepted International Drivers licence and own vehicle for work purposes or the willingness to maintain a driver’s license and own vehicle.
  • Satisfactory National/International Police Check.
  • Working with Children Check or the willingness to apply for one.
  • As a Disability/Aged Care Support worker, you should demonstrate empathy and compassion skills and have a warm and caring attitude.
  • Commitment to using a human rights-based, person-centred approach in working with participants, and always maximising their choice and control.
  • Demonstrated high-level interpersonal skills, with the ability to communicate with and relate well to people we support and their families, especially those with diverse communication styles and needs.
  • Demonstrated ability to work effectively with others as part of a team and contribute to team goals
Responsibilities
  • Provide personal care support and assistance for participants including those with complex needs in a safe and sensitive manner – including toileting, showering, mobility assistance, eating and drinking, behavioural support and transport.
  • Provide individual support and empowerment for people with psychosocial and other disabilities to help them achieve their goals through skill development and meaningful engagement in activities of their choice.
  • Collaborating with healthcare teams, including therapists, nurses, and other professionals, to develop and implement personalized care plans for individuals with varying needs.
  • Use a strengths-based, person-centred approach when working with all participants with disabilities and the aged care industry to promote independence.
  • Facilitate the establishing and maintaining of participants’ connections with the community, personal and social development, employment, education, and training in alignment with individual goals.
  • Develop and maintain safe, effective and professional relationships with participants, use active listening to develop trust and provide appropriate role modelling thereby supporting the communication needs of the client.
  • Assist participants with Activities of daily living, health and wellbeing, general life skills, and recreational activities as part of their care management plans.
  • Adhering to and assisting with the implementation of therapy plans designed for individuals with Dementia, behaviours of concerns, and other cognitive diagnosis.
  • Carefully monitoring and observing any changes in the physical, emotional, or behavioural condition of participants.
  • Empower participants to pursue significant goals and make a positive influence in the lives of people.
  • As appropriate, create client file notes and assist with administrative responsibilities to ensure that operations function effectively.
  • Actively ensuring that participants’ rights are respected, advocating for their needs, and helping them access services or support as required.
