Position Overview:
We are seeking a compassionate and organised Support Coordinator with a ‘can do’ attitude to join our team. The Coordinator will play a critical role in planning, coordinating, and managing services for individuals with disabilities, ensuring they receive the necessary support to lead fulfilling lives.

• Develop, implement, and monitor individualised service plans.
• Coordinate services and supports across various providers and agencies.
• Advocate for clients’ needs and ensure they receive appropriate services.
• Conduct regular assessments and reviews of clients’ progress.
• Maintain accurate and up-to-date records and documentation.
• Collaborate with clients, families, and multidisciplinary teams to ensure comprehensive care
