Finders Keepers WA is a local NDIS provider offering Community, Employment and Capacity buidling Supports.

Job Description: As a Community Support Worker, you will play a vital role in assisting individuals in their homes or within the community to lead fulfilling and meaningful lives. Your primary responsibility will be to provide person-centered care, promoting independence and social integration while offering practical assistance and emotional support.

Responsibilities

KEY RESPONSIBILITIES:

Individualised Support:Develop strong relationships with clients and their families to understand their unique needs, preferences, and goals. Daily Living Assistance: Assist clients with activities of daily living, including personal care, meal preparation, medication management, and household tasks.
Social and Recreational Activities:Organise and participate in community outings, events, and recreational activities to promote social interaction and community engagement. Emotional Support: Provide companionship, active listening, and encouragement to clients, helping them to build self-confidence and resilience. Support Planning: Collaborate with the care team to create personalised support plans, monitoring progress and adjusting strategies as required.
Advocacy: Champion the rights and interests of clients, ensuring they have a voice in decisions affecting their care and well-being.
Crisis Management: Handle challenging situations with sensitivity and professionalism, taking appropriate actions to ensure the safety and security of clients. Documentation: Maintain accurate and detailed records of support activities, incidents, and progress reports according to organizational guidelines.
Qualifications and Requirements:

Prior experience in community support work or a related role

  • NDIS workers screening check or willingness to obtain.

Empathy, patience, and strong interpersonal skills are essential for building positive relationships with clients and their families. Excellent communication skills, both verbal and written, to interact effectively with clients, colleagues, and community members. Flexible availability for working hours, including evenings and weekends, to meet the needs of clients. Commitment to promoting diversity, inclusivity, and respect within the community. Ability to work independently and as part of a team, collaborating with other professionals to ensure comprehensive care.
