About the Role

Cosy Home Care is seeking an experienced and compassionate Support Care Worker to join our Supported Independent Living (SIL) team in Heidelberg West.

This role involves supporting adult participants with complex mental health needs, behaviours of concern, diabetes, and a history of seizures. You will provide person-centred support to promote the participant's independence, health, safety, emotional wellbeing, and community participation while following individualized care plans and risk management strategies.

We are looking for someone who is patient, resilient, emotionally mature, and committed to delivering high-quality, person-centred care while maintaining a safe and supportive environment.

Active Shifts:

• Monday: 2:00 PM – 4:00 PM (Community Access shift)
• Tuesday: 2:00 PM – 4:00 PM (Community Access shift)
• Wednesday: 2:00 PM – 4:00 PM (Community Access shift)
• Friday: 10:00 AM – 1:00 PM (starting November onwards)
• Wednesday: 12:00 PM – 6:00 PM (1:1 Support)

Duties in this role include:

• Providing in-home assistance and supports to participants
• Assisting with personal care tasks such as showering, toileting, dressing, and grooming
• Assisting with medication administration
• Assisting with meal preparation and feeding as required
• Assisting clients with challenging behaviours
• Supporting participants in developing essential life skills and achieving their goals
• Assisting with transport for appointments and shopping
• Promoting community engagement and social inclusion
• Documenting participant progress notes and records
• Manual handling depending on client complexities
• Adhering to the participant’s care plan and NDIS frameworks
• Assisting with domestic tasks such as general cleaning and laundry

To be successful in this role, you’ll need:

• Valid Right to Work in Australia
• Strong communication skills and a positive attitude
• Minimum 1 year experience as a Disability Support Worker
• Minimum Certificate III in Individual Support, Disability, Personal Care or above
• Experience working in a SIL house
• Experience assisting complex clients with behavioural issues
• Experience working with clients with mental health and behaviours of concern
• Strong mental health background
• NDIS Worker Screening Check (required)
• First Aid Certificate HLTAID011 or HLTAID012 (required)
• CPR Certificate HLTAID009 (required)
• VIC Issued Driver's License and access to a reliable vehicle (required)
• Comprehensive car insurance (required)
• Evidence of COVID vaccinations
• Willingness to complete compliance requirements and certifications once successful
• Can start immediately

Pay: $36.00 – $57.32 per hour
