About the role

In this role, you'll support our clients in their homes and in the community. You will be responsible for various activities including in-home support, personal care, transport, and social & community access.

Responsibilities and duties may include:

• Support clients to participate in age-appropriate activities and tasks
• Provide direct care assistance to each client in accordance with their service plan and as directed.
• Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes as required.
• Work with clients, family/ carers, relevant case managers and other professionals to support the client's individual goals.
• Personal care is provided to clients in a caring and supportive way.
• Be available for ~ 20 - 30 hours fortnightly

About you

You must have:

• Certificate III in Individual Support (Home and Community Care, Mental Health or Disability) or equivalent
• A current Provide First Aid Certificate (HLTAID003)
• A current Provide CPR Certificate (HLTAID001)
• A current National Police Clearance (Obtained in the last 12 months)
• Worker’s Screening Check or current Yellow Card
• Blue card
• NDIS Worker orientation (or willing to obtain it)
• Reliable transport
• Experience with disability and AGED CARE services would be an advantage

We offer competitive pay rates. Pay $36-$40 + 12% Superannuation

Pay: $36.00 – $40.00 per hour
