Full job descriptionStream Services is seeking applications for a compassionate and experienced Lifestyle Support Worker for a contractor based in Innisfail, QLD. This rewarding role involves providing consistent, person-centered support to a NDIS Participant who is outgoing, bubbly and social person, who loves being outside and interacting with others, she prefers to be supported by female workers. We are looking for the services to be provided by someone with a positive and happy outlook who is energetic and genuine.

We are ideally seeking an applicant who is based in Innisfail. However, suitable applicants from the surrounding region who are comfortable with a short drive to Innisfail may also be considered.

The service is provided on a 7 on / 7 off rostering arrangement, meaning the Lifestyle Support Worker will live and work with a Participant for 7 days, then have 7 days off on a rotation basis with another Lifestyle Support Worker. Uninterrupted supports will be during the day and evening, including sleepovers, leading to stability, consistency and continuity of supports for the participant.

Please note that expressions of interest would be for already established PTY Ltd. companies, sole trader ABN holders or Lifestyle Support Workers seeking to set up an ABN in order to trade independently for services.

The total remuneration is $169,103 per annum salary, plus superannuation entitlements.

What You’ll Do

• Support the Participant with daily living activities, including personal care, meal preparation and household tasks.
• Provide social and recreational support to build confidence, independence and community participation.
• Follow individualised support plans aligned with the Participant’s goals and needs.
• Maintain accurate, NDIS-compliant documentation and reporting.
• Work independently while collaborating with the wider support team to deliver consistent, high-quality care.

About You

We’d love to hear from you if you have:

• Qualifications in disability support, aged care or community services, or equivalent experience as a carer.
• Experience providing person-centered support to NDIS or aged care participants.
• A warm, empathetic and professional approach.
• Strong communication skills and the ability to build positive relationships.
• A commitment to upholding the rights, choices and dignity of people with a disability.
• The ability to work independently and maintain consistency during the rostered engagement.
• Current First Aid and CPR certificates, or willingness to obtain them.
• A NDIS Worker Screening Check and Blue Card, or willingness to obtain them.
• A valid driver’s license and full working rights in Australia.
