We are seeking a dedicated and compassionate Community Worker to join our team at Amity WA on a full-time basis, based in Leederville, WA 6007. In this pivotal role, you will provide essential support and assistance to individuals within the aged and disability support sector. As a Community Worker, you will be instrumental in enhancing the quality of life for vulnerable clients by delivering personalised care, facilitating community engagement, and promoting independence and social inclusion.
Key responsibilities
- Provide direct support and assistance to clients in the aged and disability sectors, helping them with daily living activities and community participation
- Develop and maintain supportive relationships with clients, understanding their individual needs, goals, and preferences
- Assist clients in accessing community resources, activities, and social opportunities to promote engagement and inclusion
- Support clients with practical tasks such as shopping, meal preparation, personal care coordination, and mobility assistance
- Liaise with families, carers, and other professionals to ensure coordinated and holistic support
- Maintain accurate and confidential client records and documentation in compliance with organisational policies
- Identify and report any concerns regarding client welfare or safeguarding issues to appropriate supervisors
- Participate in team meetings and contribute to the continuous improvement of services
- Demonstrate commitment to person-centred care principles and uphold the dignity and rights of all clients
- Comply with all relevant legislation, including health and safety, disability discrimination, and privacy requirements
