DISABILITY SUPPORT WORKER AT LEDA SUPPORT SERVICES PTY LTD

Location: Kippa-Ring or Boondall
Employment Type: Casual (Part-time or Full-time offered after 12 months)
Pay: $33.00 - $36.00 (DOE = Depending on Experience)

THE ROLE

We are seeking a dedicated and organised Disability Support Worker to join our team. This role is ideal for someone who values structured processes, consistency, and teamwork.

KEY RESPONSIBILITIES

• Support clients to participate in age-appropriate activities, employment, in-home support, and community access.
• Provide direct care assistance in line with individual service plans.
• Administer medication safely in accordance with policy and procedures.
• Monitor client wellbeing and report any concerns to management.
• Deliver 1:1 or small group training and support to help clients achieve personal goals.
• Complete accurate documentation, including client notes, daily records, and reports.
• Collaborate with clients, families, carers, case managers, and other professionals to support individual outcomes.
• Contribute to the development, implementation, and evaluation of training and behaviour programs.
• Act as a key worker for designated clients, including preparing monthly progress reports.
• Support team operations through effective communication, attending meetings, and assisting colleagues when required.
