Key responsibilities

• Support clients with personal hygiene, grooming, dressing, toileting, mobility and other daily living activities while maintaining their dignity and privacy
• Deliver person-centred services according to individual support plans, preferences and personal goals
• Assist clients in accessing community activities, social events and recreational opportunities to promote inclusion and engagement
• Support clients with meal preparation, shopping, cleaning, laundry and other household tasks to maintain a safe and comfortable living environment
• Monitor clients' general wellbeing, recognise changes in their needs and report concerns to relevant supervisors or healthcare professionals
• Assist clients in attending medical appointments, therapy sessions and other essential engagements
• Encourage and support clients in developing daily living skills, confidence and independence
• Maintain accurate records of support activities, client progress, observations and incidents in accordance with organisational policies
• Work closely with clients, families, carers and other professionals to ensure coordinated and consistent care
• Maintain a safe environment by following infection control procedures, manual handling guidelines and workplace health and safety requirements
