Overview:
Schedule: PRN; Hours available any day of the week between 8:00 AM - 6:00 PM; must be available to train during the week for several weeks
Location: Charlotte Orthopedic Hospital - Charlotte, NC; after 6 months will move to a hybrid schedule

• Serve as facilitator of the patient throughput/discharge planning process
• Collaborate with case managers and other members of the healthcare team to implement the discharge plan agreed upon by the RN, case manager, physician, patient/chosen support system, and other involved members of the interdisciplinary team
