We are hiring full time Discharge Planner  to our join our Inpatient Team. This position is Monday – Friday, 8:00 am – 4:30 pm.  Wellfound Behavioral Health Hospital is a 120-bed inpatient psychiatric hospital located in Tacoma, Washington.  We are Department of Health and Joint Commission accredited. We are an acute care psychiatric hospital providing care to both voluntary and involuntary patients. Position information:The primary role of a Discharge Planner for behavioral health is to act as a liaison between the patient, the interdisciplinary treatment team, and community resources, and to collaborate with the patient, identified supports and interdisciplinary treatment team to address social determinants that are barriers to healthy living for discharge. Beginning on the day of admission, the Discharge Planner will document recommendations for discharge plan or care transition and will document efforts made toward planning a safe discharge.   The Essential Job Functions Include: * Collaborate with patient and interdisciplinary team to develop a treatment plan. * Assess and screen patients for resource eligibility and schedule post-discharge appointments. * Assist in obtaining pre-authorizations/pre-certifications, insurance eligibility and authorizations for medications, hospital stay, community needs as needed to support the patient during hospitalization and upon discharge. * Attend treatment team daily and coordinate the patient care with other interdisciplinary treatment team members. * Communicate effectively with treatment team members and others regarding patient needs, and current issues. * Assist patients with transportation needs, if needed, for discharge after vetting out any potential options. * Support patient through their hospital stay through weekly outreach to identified outpatient supports regarding discharge-related planning and updates. * Identify resources available to meet patient needs after discharge. * Facilitate group education for patients regarding how to access community resources after discharge. * Assist in monitoring and directing the milieu as requested by treatment team members. What We Require:  Education: * Bachelor’s degree in behavioral science or related field License: * Basic Life Support (BLS) card through American Heart Association or American Red Cross required  to start Experience: * One year of related work experience in the mental health field strongly that demonstrates attainment of the requisite job knowledge skills/abilities  Union Membership As a condition of employment, you must join the SEIU Union within thirty (30) calendar days of employment and remain a member in good standing of the Union, unless you submit and are approved for a religious exemption.
